From context to a decision

A decision pack makes a choice inspectable. State the question, source facts, assumptions, options, risks and proposed next step. Link the evidence behind material claims.

Keep the human decision and rationale separate from the model's proposal. Use multiple perspectives when they could reveal a real disagreement; several agents repeating one answer are not independent validation.

After the decision, record what was applied and how it was checked. A later review may improve the method but must preserve the original evidence. Do not claim causal improvement from a before/after comparison without considering other changes.
